Eligibility

A) Children ages 3 to 18 B) Provides services for any child, regardless of their family's ability to pay and services are provided on a self-pay sliding fee scale if the child does not qualify for Medical Assistance

Hours

Monday through Friday, 8:00am to 5:00pm

Call to make an intake appointment. Appointments are Monday through Thursday.
