Provides children, transitional youth, and parents a child-centered service designed for children and adolescents who have, or are at risk, for a serious mental illness or emotional disturbance, get the treatment and support necessary to help them succeed in the home, at school, and in the community.

Eligibility

Serves Erie County residents Child service requirements: up to age 18 or up to 21 if the child is still in school and has an individualized education plan (IEP) and enrolled in school. Transitional Youth requirements: age 18-24 who meets criteria for either child or adult eligibility based on age and school status. Parents requirements: adult over 18, has a diagnosis of a serious mental illness (e.g. schizophrenia, bipolar disorder); recent psychiatric hospitalization or frequent contacts with Crisis Services; and significant difficulty independently management responsibilities at home, work, in the community.
