Eligibility

A) Individuals and families with a sustainable source of income (excluding county assistance or social security) B) Able to physically live and function independently C) Homeless or facing eviction D) Homelessness or potential eviction must have originated in 17022, 17033, 17036, or residing in one of these areas for at least 9 months

Description

Provides temporary housing for up to 12 months while program members participate in the transitional housing program that includes case management and addresses financial, employment, physical, mental health, and spiritual needs. An additional 6 months of supportive services are provided during transition into permanent housing.
