Eligibility

For Medical Services: Student must be a patient of Hamilton Health or have no other health care provider & must have a signed consent from their parent to be seen in the clinic. If student has a regular health care provider and is sick, their regular health care provider can give permission for a visit to SBHC. For Dental Services: A signed consent form must be returned (available on HBG School website: www.hbgsd.k12.pa.us)
