Intellectual and Developmental Disabilities Supports Coordination
Provides case management services for people with Intellectual and Developmental Disabilities. These services are provided both in the home and the community. People may reside in a residential (group home) setting, in their own home or family member's home, a state center or a nursing home.

Application process
Intake and assessment of eligibility is completed at the local county level.
Fee
If a person resides in a residential home or lives with their family, that person is responsible to remit payment to the agency or homeowner for room and board. This cost is 72% of the person's Social Security income.; None
Eligibility
Must have an IQ of 70 or below and diagnosed with a developmental disability (mental retardation) before the age of 22. Documentation must be provided. The person must also have a current physical indicating this diagnosis, as well as an “ICF/MR” level of care indicated in order to be eligible for services.
