Call to schedule a trip and be prepared to provide the following: A) full name B) appointment day and time C) destination arrival time with destination address and name D) if a mobility device is being used E) if an escort is coming F) time of pickup

Co-pays are on a sliding scale. Fees based on mileage. Call for more information
