Must be at least 18 years old, must be able to be named on a lease as a
responsible party, must have CCBH (Medical Assistance) Insurance, must
have a diagnosis of a serious, persistent mental illness and/or
substance abuse, must have some source of verifiable income within the
household.
Monday through Friday, 8:00am to 5:00pm
Applications for assistance are accepted during open enrollment periods, approximately 4 times per year. Emergency applications are accepted at any time.
