Visit the website to download the Request for Taxpayer Advocate Service Assistance (IRS Form 911) which is linked in the Intake Website below. It can be returned by fax or mail. Call for additional assistance or for help determining eligibility.

Eligibility

Serves individuals, families, and businesses who cannot resolve federal tax issues through the IRS on their own. Use the TAS Qualifier Tool (linked in the Eligibility Website below) to determine whether the Taxpayer Advocate can help with the issue.
