Facility: 24/7 Office: Monday through Friday, 8:00am to 4:30pm

Telephone, internet. Residents may be referred by a physician, insurance company, discharge planner/social worker, home health agency, family member, or self-referral.

Offers intermediate and skilled nursing care for persons with physical disabilities. A 99-bed facility, the Raker Center is open to residents 12 years old or older with primary diagnoses including, but not limited to, cerebral palsy, muscular dystrophy, quadriplegia, or Multiple Sclerosis. Services include daily occupational therapy activities; pastoral care including linkages with outside congregations, weekly ecumenical worship, and religious education program; physical therapy services designed to improve physical function; speech and language therapy; and recreational therapy designed to provide residents with a well-rounded lifestyle.
