Must be 18 years or older with a chronic psychiatric illness and a
history of long-term hospitalization and/or have been identified by
BH/DS as being at risk for inpatient of state hospital services, no

Provides short term, intensive and individualized treatment services for the identified persons in order to successfully re-integrate them into the community. Facility consist for two 3 bedroom semi-detached homes and accommodates up to seven individuals. Program length ranges from 6-12 weeks depending on individual's needs.
