Children ages 3 to 21, who are still in school and who receive Medical Assistance benefits and have a mental health diagnosis: severe emotional, behavioral, and/or mental health problems

Referral from PerformCare, Community Care Behavioral Health or County Base Service Unit; Must have a psychological or psychiatric evaluation which recommends the service
