Eligibility is based on the charity care policies of each of the local
health systems. Federal poverty guidelines used to determine financial
eligibility.

Application process
Complete an application. A person must enroll in health insurance if eligible for it. (For example: Medicare, Medicaid, or commercial insurance through a job. A completed application with proof of income and assets will determine what discounts a person may be eligible to receive. A person with the HCC and no other prescription coverage will be given a list of pharmacies where prescriptions can be dispensed at a discount.
