Eligibility
Eligible clients are adults and youth, age 14 and older, residing in Westmoreland County and Allegheny County who have a functional impairment that inhibits them in their lives. Enrollment requires a recommendation from a Licensed Practitioner of the Healing Arts. A LPHA is defined as: Medical Doctor, Nurse Practitioner, Licensed Psychologist, Doctor of Osteopath, Physician's Assistant. In order to be eligible, a person referred must be struggling with an impairment due to their mental health condition that limits their attainment of goals in the areas of: Living, Learning, Working, Social, or Wellness. Insurance is required: Medicaid (Health Choices) and both Westmoreland and Allegheny County Fee for Service.
