Description

Promotes self-sufficiency, self-determination, and stable community living among consumers of mental health services. Provides individuals with access to needed services and supports. Acts as an advocate on behalf of consumers. Includes two levels of Administrative Service Coordination (ASC) and Targeted Service Coordination (TSC). Administrative Service Coordination is available as needed for individuals who are managing community living on their own. Targeted Service Coordination is available to those who have more complex needs and challenges with community living.
