Helps the caregiver by providing information about services that are available through State and Federal Caregiver Support Programs. Helps by locating care providers, reimbursing for services and assistance with home modifications or assistive devices. The program empower the older adult and his/her caregiver to
choose what services suit their needs the best so that service is individually tailored for each family.
