A) Documentation required to process the application: B) Copy of child’s birth certificate or other proof of age, C) Proof of income (at least 2 pay stubs, W2 form, tax form 1040, TANF, SSI, child support/alimony, signed and dated statement from employer, and/or social security). If family receives SNAP benefits, they can use proof of that to determine eligibility.
