The Elks home nurse can provide the following services for a person with a developmental delay: - in-home medical case management services - advocate for an individual's legal and human rights - provide information regarding resources and services available - assess needs and help to access equipment - advocate for appropriate educational services - assist with finding activities within the community - offer instruction regarding home care - provide emotional support - help with vocational goals - assist with residential placement - provide referrals to appropriate agencies - interpret clinical findings and act as liaison between health care professionals

Application process
Referrals can be made by family member, health care professional, community agency, or friends. After referral, nurse will obtain permission, visit home for intake and initial assessment. Nurse will work with individual and family to create a plan which may include referrals, follow-up visits, and home visits as needed.

Any individual of any age who has a disability may request services. The disability had to have manifested before a person is 22 years of age. It may include a physical or mental delay or a combination of both.
