Referral required from Coordinated Homeless Outreach Center (CHOC) or from other agencies working with the homeless population as long as they meet the eligibility requirements.

Description

Critical Time Intervention (CTI)/Case Management it is an evidenced-based practice proven to reduce the number of people returning to institutions once released into the community. Case management for homeless invdividuals with mental illness is provided.

Provides services to adults and children with mental illness, developmental disabilities, substance abuse and those who are homeless. Programs include therapy and counseling services, job training, assisted transportation, crisis intervention, and supportive and/or affordable housing.
