Description

Provides elderly or chronically ill veterans with coordinated care that focuses on individual needs, enabling each patient to enjoy the highest quality of life possible. Provides assessment, prevention, stabilization and rehabilitation to eligible residents. Services that are provided fall into the following categories: Hospice and Palliative Care, Home Health Care and Adult Day Health Care, Community Living Center Care and our outpatient Golden Memory Clinic.
