Provides hospice and palliative care wherever patients feel most comfortable, whether in their home or in a senior living community. The program offers expert assistance with symptom management and ensures continuity of care throughout the patient's journey.
The hospice team includes the medical director, the patient's primary care physician, registered nurses, medical social workers, home health aides, and specially trained volunteers.
Also offers spiritual and bereavement counselors, and a variety of holistic therapies including pet therapy, aromatherapy, and Reiki to enhance patients' comfort and overall well-being.
