This 4 week service is free and is in conjunction with Wayne Memorial Hospital.

Aims to help people who are discharged from a health care facility. Following discharge, patients
often require care from different medical professionals in many different settings. This program is designed to improve the continuity of care for persons with chronic conditions across different health settings and ensure that each transfer between settings goes smoothly.

Oversees the administration of all County human service agencies, acts as a coordinative unit for all County contract human service agencies, and facilitates the access of citizens to service coordination.
