Eligibility
Individuals ages of 18 to 65 and do not qualify for Medicaid or other government programs.
Total household income does not exceed 250% of the Federal
Poverty Guidelines (FPG).
Employer does not provide medical or dental insurance; or cannot afford insurance premiums offered through the private or public marketplace.
