Eligibility

Individuals ages of 18 to 65 and do not qualify for Medicaid or other government programs. Total household income does not exceed 250% of the Federal Poverty Guidelines (FPG). Employer does not provide medical or dental insurance; or cannot afford insurance premiums offered through the private or public marketplace.

Monday through Friday, 9:00am to 4:00pm Appointment required.

Provides free medical care to uninsured individuals. Services include routine medical visits, chronic disease management, health screenings, and medication management. Specialty care available includes cardiology; pulmonology; orthopedics; dermatology; ear, nose, and throat; endocrinology; gynecology; ophthalmology; physical therapy; and rheumatology.

Provides intensive case management for community members, food for the hungry, housing for the homeless, free health care for those who cannot afford it, help for pregnant women and the parents of young children, utility assistance, counseling services, care for the elderly, and more.
