history of difficulty maintaining independent living
Hours
Monday through Friday, 8:00am to 5:00pm, on call 24/7

Provides services to maintain independent living. Services provided in the home or community. Supports include: use of community resources, development of independent living skills, financial and medical insurance assistance and healthcare services. Support is provided on an hourly basis.
