Disabilities & Health Conditions

  • Developmental Disabilities
A state and county funded agency mandated by law to provide services for persons with intellectual disabilities. Works closely with many other agencies, bureaus and providers of these services. Manages the State Medicaid Waiver Program. Supports Coordinators assist individuals with intellectual disability to connect with various services:
  • Family Support Services: respite care, family aide, nursing care, home and community habilitation
  • Residential Services: supervised apartments, semi-independent living, group homes, family living/life sharing
  • Vocational Training: Job skills, obtain and maintain a job, workshop setting, transitional employment, volunteer work, job search and placement, supported employment, job loss prevention
  • Adult Day Services: Adult developmental training focusing on basic life skills and leisure activities, community habilitaion, senior programs
