Child or adolescent with a Serious Emotional Disturbance (SED): The youth must have a diagnosed mental health condition causing significant functional impairment. At risk of out-of-home placement: The child must be at risk of psychiatric hospitalization, residential placement, or other out-of-home care without intensive in-home intervention. Family participation required. Clinical and service necessity determination. Age range: Typically serves children and adolescents under age 21 (exact age limits may vary slightly by provider and payer authorization).
