Eligibility
  • Foster to Adopt Services: Any child within custody of Children and
Youth services - Adoptive Families: 21 years or older, live in PA, completion of PA child abuse, criminal record and FBI clearances and completion of a family profile - Post Permanency Services: Adoptive Families who have provided permanency through adoption, guardianship or kinship and who live in PA
Hours
Monday through Friday, 8:30am to 5:00pm
Application process
  • Foster to Adopt Services: Through Children and Youth services who has custody of the child
  • Adoptive Families: Self-referral, Children and Youth Services if child placed in their home
  • Post Permanency Services: Self-referral to SWAN Helpline - 800-585-7926

Designed to offer birth families, the child/adolescent, and the adoptive/permanency parents the availability of counseling and services need for the adoption or permanency process including direct interactions and/or a referral. Provides a continuity of services for children in care whose placement goals are identified for permanent living arrangements including reunification, kinship, adoption, or legal custodianship. Services consist of a Family Profile, Child Profile, Child Preparation, Placement/Finalization Supervision, Child Specific Recruitment, Support Group, and/or post Permanency Services. Fully licensed PA Adoption Agency and a Statewide Adoption and Permanency Network (SWAN) Affiliate.
