-Only people with COVID-like symptoms will be tested at these facilities. -A medical order for testing is strongly encouraged. -Bring your personal ID and insurance information when being tested. Most insurances are accepted.

Please contact your primary care provider for assessment or call the LVHN nurse triage line at 888-402-LVHN (5846) prior to coming to our Assess and Test locations. You may also do an online screening at LVHN.org/virtualcare.
