Assists individuals to fully function as members of our community with-out twenty-four hour staff supervision. A Supported Living Coach provides supports to individuals in one or more of the following areas: scheduling appointments, budgeting, shopping, transportation, etc. Family Living/Life Sharing offers an opportunity for an individual with intellectual and developmental disabilities and a family or individual to share their lives. Family Living/Life Sharing is based on the importance of enduring and permanent relationships as the foundation for learning life skills, developing self-esteem and learning to exist in interdependence with others.

Offers residential programs, community-supported living programs, an adult day program, in-home services, supported employment training, and transportation to work sites.

Offers services for personal care needs such as bathing, dressing, activities of daily living, health care management for medical needs, and medication administration. Transportation and activities programs are available. Meals provided for dietary needs.

Provides residential program for individuals with mental illness. - Long Term Structured Residence (LTSR): Residential treatment program that provides treatment and rehabilitation services in a structured environment for adults with a severe or chronic mental health need.

Supported housing for persons who suffer from chronic mental illness. Live independently in the community while receiving the supportive assistance they need through regular in-home visits. Includes: * budgeting/money-management * housekeeping * nutrition/meal planning/cooking * shopping skills * medication management * leisure time use * socialization * community participation * risk management/safety * mobility/transportation * wellness management

Assessments for care and eligibility will be done. -Nursing Home Assessment: Any person age 18 and older who desires placement in a nursing facility, personal care home, or domiciliary care home can be assessed by the Care Management staff of the Adams County Office for Aging, Inc. -Domiciliary Care (Dom Care): Provides adults 18 years or older, who cannot live independently, with sheltered living in private homes with a family-like atmosphere. -Personal Care Homes: Sometimes called assisted living facilities, offer room and board and assistance with daily living activities. -Adult Day Services: Provides functionally disabled older persons with an array of activities in a protected environment which will enable them to maintain independence within the community.

Offers a no-cost, multi-faceted lifestyle evaluation to determine the programs, services, and support available to assist you to Live Well with a Disability. Provides programs, services, and support to Live Well with a disability.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Provides older adults with shelter, food, medical support, and physical therapy.

Provides 14 structured group home opportunities for individuals needing intensive support and specialized housing accommodations in order to live successfully within the community.

Senior living community. Provides enhanced care, and memory care needs. Activities and programs are offered. Services include meals, laundry and housekeeping, medication management, assistance with ADL's, activities, etc.

Provides a variety of supports to individuals with intellectual and developmental disabilities to enhance their abilities and interests. Services include supported living, in-home supports, day programs, group living, intermediate care facilities, supports coordination, and respite care.

Provides varying levels of care including palliative care, hospice care, and general nursing care.

Offers family-style residential homes, planned short term respite, home and community supports, companion services, life sharing, supported living, advocacy, volunteer opportunities, support for families and caregivers, shared lives, and spirituality.

The Transitional Care Center for Mental Health is a long term structured residence for individuals with mental health needs.

Provide Community Residential Programs to adults with intellectual disabilities who choose to live in a community setting.

Provides care and assistance in all aspects of daily living, as well as a variety of social, spiritual, and educational programs and activities. Memory and short term care available.

Residential group homes services for persons with intellectual disabilities. Operate homes with 1 to 4 people in each setting. Supports and services are provided.

Provide the opportunity for many individuals that need extra care to live in a home environment with the support of 24-hour staff to assist in daily activities. Creates an environment in which each individual can learn to increase their independence while socializing with peers and in the community.

Provides homes and a full range of services to individuals with all levels of intellectual and developmental disabilities (IDD). Operates 106 community group homes and 4 campuses across Pennsylvania. Develops individualized plans of care for each person with the goal of achieving his/her highest level of independence while enhancing quality of life. Services range from physical, occupational, and communication therapies to vocational training and employment support. Therapeutic activities and developmental training focus on sensory, environmental, and pre-vocational skill development.

Helps consumers to stay in their homes longer and more comfortably as an alternative to placement in a nursing facility. Provides care managers that assess consumer needs, arrange for services, and monitor the care provided.

Provides assisted living services, including laundry, housekeeping, prescription delivery, transportation, mobility assistance, and help with daily tasks (eating, bathing, dressing, toileting, hygiene, grooming).

Personal care home, which includes spacious apartments in four different layouts. Apartments include, privacy and security, private bath, kitchenette, an emergency help system, and an individual parking space nearby if need be. Chapel Ridge offers, family style meals in our restaurant style dining, on-site beauty salon, in house laundry, and housekeeping services, as well as weekly outings including shopping, banking and many engaging activities. Also, caring staff are available on site 24 hrs a day.
