Provides residential programs for individuals with mental illness. - Community Residential Rehabilitation (CRR): Provides recovery and resiliency based, independence oriented, life skill and community integration activities and support for adults living with mental illness in the Lancaster Community. Provides programming, support and transitional housing for helping individuals acquire skills needed for independent community living. Offers apartment living supervised by trained staff. Offers skilled nursing and rehabilitation services in a nursing home setting. Provides a full range of therapy services, short and long term care, and acute care.

Provides a community living home for individuals with developmental disabilities. Program focuses on helping individuals live successfully and independently in the community. Support services include adult training, behavior management, individual program planning, communication skills training, and money management instruction.

Lifesharing provides people with disabilities with an opportunity to live as part of an extended family in the community. Participants are given the choice of where and with who they want to live.

Personal Care

Provide Community Residential Programs to adults with intellectual disabilities who choose to live in a community setting.

Provides living arrangements in community residences for chronically mentally ill adults and selected children. Operated by providers under contract to the agency, there are several types of programs and multiple sites. Residential services include community residential facilities (CRF), community residential rehabilitation (CRR) services, long-term structured residences (LTSR), and residential treatment facilities (RTF).

Offers independent living, personal care, and nursing care.

Supported housing for persons who suffer from chronic mental illness. Live independently in the community while receiving the supportive assistance they need through regular in-home visits. Includes: * budgeting/money-management * housekeeping * nutrition/meal planning/cooking * shopping skills * medication management * leisure time use * socialization * community participation * risk management/safety * mobility/transportation * wellness management

Offers personal care, respite care, and memory care in a senior living facility setting.

Provide assistance and supportive services for participants in eligible housing developments.

Adult Day Programs - Day programs are created to meet each individual’s needs and skill set. Both licensed and unlicensed day programs are held in a safe and caring environment with a team of human service professionals. Adult Residential Programs - Support is provided to individuals in community homes or life sharing with a host family. Each individual is evaluated to determine which living environment will work best for them depending on their needs, wants, and goals. Behavioral Services - Behavior support services use empirically-validated practices to identify the causes of, intervene to prevent, and create appropriate reactions to problematic behavior. Vocational Training - Vocational Training provides the supports individuals need to successfully find and maintain a job. Individuals receive assistance to build their resume and discover their skills and abilities. Companionship Services - A companion is a Direct Services Professional that provides support in the individuals own home while

Provides low-income/ subsidized private rental housing for individuals experiencing chronic mental illness.

Provides community homes which offer a home-like setting while meeting the needs and unique challenges that individuals may face on a daily basis. The program is designed to provide personalized care from age 21 through end of life. Each individual’s specific health requirements are accommodated, along with specialized nutritional, therapeutic, behavioral, educational, employment, and day programming needs.

Provides a supportive environment to assist individuals with mental health challenges to learn independent living skills. Services are provided in a group home setting. Teach life skills.

Provides residential services for individuals with intellectual and developmental disabilities. Options include group homes and lifesharing.

Provides four types of assisted living: Personal Support I; Personal Support II; Health Support; and Memory Support. Assists with meals, housekeeping, laundry services, help with dressing and bathing, medical coordination, medication ordering/management, emergency call system, and other personal needs.
