Residential setting assists individuals with learning skills in 11 skill area. Individuals share apartment with 2 other peers. - Supported Housing

Provides community homes which offer a home-like setting while meeting the needs and unique challenges that individuals may face on a daily basis. The program is designed to provide personalized care from age 21 through end of life. Each individual’s specific health requirements are accommodated, along with specialized nutritional, therapeutic, behavioral, educational, employment, and day programming needs.

Provides independent and personal care residential options.

Places individuals into the home of community providers for long-term care and training. Individuals in this program are totally integrated into the daily lives of the families with whom they reside.

Provides low-income/ subsidized private rental housing for individuals experiencing chronic mental illness.

CSG offers a wide range of support for individuals with intellectual and developmental disabilities. Services are individualized and driven by the needs and goals of the individual and his or her loved ones, with a focus on increasing independence at home and in the community. Support includes: Residential living arrangements; Supported care for individuals who live on their own or with their families; Employment services that provide the guidance needed to find and retain jobs; Adult day programs in both group and individual settings; Behavioral support services.

Provides residential programs for individuals with mental illness. - Enhanced Personal Care Home (EPCH):Provides residential services to adults with chronic mental health conditions, offers community living opportunities, supports and assists residents with personal care needs and access to community services, including social and/or personal adjustment. Areas of need focused on are personal hygiene, daily living skills, medication management/administration, community social services, recreational activities, health services, laundry services and dietary needs. 16-bedroom facility with a home atmosphere with a fully functional kitchen, dining area, living room, activity room, sitting area, multiple full and half bathrooms and a front yard and back yard with patio furniture included.

Provides a personal care home to seniors and adults with disabilities. Includes meals prepared at the Center and served family style, the administration of medication, and a daily schedule of planned activities.

Provides licensed homes in the community with 24/7 staff member support. Assistance is provided withmedications, meals, housekeeping, finances, decision-making, health and wellness, and emotional well-being. The Residential Program is offered on a long-term or a short-term basis. Supported Living program enables the individual to remain at home with support from staff to manage personal resources, health, mobility, transportation, and use of community resources to promote independence.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Lifesharing programs enrich the lives of individuals with intellectual and developmental disabilities by matching them with a long-term support family. By living together and sharing life experiences, lifesharing programs promote individual growth and cultivate supportive relationships.

Provides a supportive environment to assist individuals with mental health challenges to learn independent living skills. Services are provided in a group home setting. Teach life skills.

Offers a range of services for adults with developmental disabilities including respite care, pharmacotherapy, in-home supports, foster care, and day programs. Also provides individual, group, family, and marital therapies.
