Supervised private family homes provide alternative family living arrangements for adults with developmental disabilities who need supervision and personal care and who do not live with their own families.

Operates six Community Living Arrangements in Cambria County. Offers activities of daily living, decision making, and social and emotional growth that encourage individuals to take charge of their lives with help available when needed.

Provides services to maintain independent living. Services provided in the home or community. Supports include: use of community resources, development of independent living skills, financial and medical insurance assistance and healthcare services. Support is provided on an hourly basis.

Provides a variety of supports to individuals with intellectual and developmental disabilities to enhance their abilities and interests. Services include supported living, in-home supports, day programs, group living, intermediate care facilities, supports coordination, and respite care.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Provides community living for people with developmental disabilities through placement with a host family. Person-centered training for the foster family and the participating individual focuses on everyday living skills, family dynamics, and skill development.

Provides a safe, healthy and home-like living environment filled with choice, for all individuals residing with them. Features residential services, adult training facility, and educational services.

Provides information on Home Delivered Meals, In-Home Services, Personal Emergency Response Systems, Senior Housing, Transportation, Adult Daycare, Prime Time Health, Caregiver Support, Insurance counseling, PACE, Tax/Rent rebates, Senior centers, Volunteer Opportunities, Protective Services, Nursing Home Transition, Level of Care Assessments, Medicaid Waiver program, or any other issues of concern to senior citizens.

Provides living arrangements in community residences for chronically mentally ill adults and selected children. Operated by providers under contract to the agency, there are several types of programs and multiple sites. Residential services include community residential facilities (CRF), community residential rehabilitation (CRR) services, long-term structured residences (LTSR), and residential treatment facilities (RTF).

Provides an ambulatory assisted living residence for older adults.

Provides semi-Independent living environment where staff support is available 40 hour a week for individuals who have already acquired the basic skills necessary for independent living, but are not yet ready to live entirely on their own. Staff assists individuals in refining their skills to the level necessary for complete independence.

Places individuals into the home of community providers for long-term care and training. Individuals in this program are totally integrated into the daily lives of the families with whom they reside.

Provides residential services to adults with developmental disabilities.

Provides a variety of supports to individuals with intellectual and developmental disabilities to enhance their abilities and interests. Services include supported living, in-home supports, day programs, group living, intermediate care facilities, supports coordination, and respite care.

Provides homes at a subsidized rate and provides supportive services to ten individuals who are homeless and who have a history of experiencing persistent mental illness. Individuals meet weekly with a case worker, help them with medication checks, makes sure they are using outpatient therapy and assists them with finding employment. Collaboration of Philhaven, Lebanon Housing Authority and Lebanon MH/MR.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Offers veterans six extended care facilities throughout the state. Care provided includes personal care, skilled nursing care, domiciliary care, and dementia care.

Provides residential programs for individuals with mental illness. - Enhanced Personal Care Home (EPCH):Provides residential services to adults with chronic mental health conditions, offers community living opportunities, supports and assists residents with personal care needs and access to community services, including social and/or personal adjustment. Areas of need focused on are personal hygiene, daily living skills, medication management/administration, community social services, recreational activities, health services, laundry services and dietary needs. 20-bedroom facility with a home atmosphere with a fully functional kitchen, dining area, living room, activity room, sitting area, multiple full and half bathrooms and a front yard and back yard with patio furniture included.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Provides a residential services program for individuals with intellectual and developmental disabilities. Also provides skill development in areas such as personal hygiene care, meal preparation, home maintenance, banking, and community membership.

Provides a variety of supports to individuals with intellectual and developmental disabilities to enhance their abilities and interests. Services include supported living, in-home supports, day programs, group living, intermediate care facilities, supports coordination, and respite care.

Provides assisted care residential services for older adults, as well as respite and short term care.

Provides housing supports to adults with mental illness and co-occurring disorders in the community through 24-hour residential care. The program aids participants in developing functional skills to support successful community living; focusing on self-care, independent living skills, medication management, interpersonal skills, and home and community safety. Participants are provided with the level of support necessary to be safe and healthy at home and in their community.

Provides assisted living care in a residential setting. Services include personal grooming, laundry services, assistance with medication, and meals.
