Liberty House provides permanent housing and supportive living services to individuals with mental illness and their families. Case management services are provided on site. Residents must be eligible for section 8 housing, and have a mental health diagnosis.

Offers affordable rental housing for low-income seniors ages 62 and older and adults ages 18 and older with disabilities. Occupancy is limited to a maximum of two persons per bedroom.

Provides residential services to individuals with 24 hour supervision and assists them in all areas of daily living and community involvement. Personalized, professional, and Direct Support Professionals (DSP) provided.

Offers residential homes and programs, supportive living, and life sharing.

Provides individuals older than the age of 18 with a chronic mental health diagnosis an opportunity to achieve personal growth and fulfillment through various treatment options. Programs include: Enhanced Care CRR (Community Residential Rehabilitation), Community Based Supported Housing, Geriatric Personal Care Boarding Homes, and a Comprehensive Mental Health Personal Care Home program. All of these programs have a goal to enhance the development of the skills, attitudes and habits necessary to prevent re-hospitalization, as well as to encourage successful community reintegration.

Offers personal care services in the home, and domiciliary care for adults ages 60 and over.

Provides disability related supportive housing and personal care facility for low-income residents with a physical or mental disability that require assistance with the activities of daily living. Services are provided including resident care, health care, meals, and social services along with many recreation and volunteer activities.

Offers senior care including independent living, assisted living, memory support and care, and medication assistance.

Provides a variety of supports to individuals with intellectual and developmental disabilities to enhance their abilities and interests. Services include supported living, in-home supports, day programs, group living, intermediate care facilities, supports coordination, and respite care.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Lifesharing provides people with disabilities with an opportunity to live as part of an extended family in the community. Participants are given the choice of where and with who they want to live.

Provides twenty-four hour personal care supportive services, assisting consumers with their Activities of Daily Living, including assistance with medication administration and bathing and dressing, whether long-term placement or a short-term care, a variety of services are available to enable consumers to achieve their highest practicable level of physical, emotional, and spiritual wellbeing.

Offers a no-cost, multi-faceted lifestyle evaluation to determine the programs, services, and support available to assist you to Live Well with a Disability. Provides programs, services, and support to Live Well with a disability.

Lifesharing provides people with disabilities with an opportunity to live as part of an extended family in the community. Participants are given the choice of where and with who they want to live.

Offers long term, residential care for individuals with disabilities. Additionally, operates multiple supervised community waiver homes. Residents hold jobs in the community and are functioning semi-independently in their home.

Works with families in sharing their homes and lives with individuals with diverse abilities who have a desire to live in a home setting. For their involvement, the Lifesharing provider receives a monthly stipend for services rendered.

Provides the opportunity for three to four individuals to learn to manage their home together with support and physical assistance from staff.

Offers veterans six extended care facilities throughout the state. Care provided includes personal care, skilled nursing care, domiciliary care, and dementia care.

Provides recovery and resiliency based support to adults living with mental illness. Housing is a very important component to recovery. Having the security of a home and the support of services helping to navigate through housing issues is beneficial. A flexible support that can help individuals obtain, maintain and/or improve their current housing situation. Services are provided in individuals' homes and in the community. 1 hour per week up to more intensive scheduling based off the individual's needs.

Offers private and semi-private rooms with skilled nursing, medication management, and personal care services.

Provides housing options for individuals with a history of, as well as current, mental health needs. These options range from independent individual apartment living, through fully-supervised group living sites in both urban rural settings.

Supported Living Program helps individuals to find housing and/or develop the independent living skills to maintain their housing. Allied Services does not have housing but helps to navigate the housing system process.
