Maintains disease surveillance records and provides the following services to individuals with tuberculosis disease, tuberculous infection or who have a high risk for disease or infection: outpatient examination and diagnostic services, and antituberculosis medication for treatment and prevention of the disease.

Provides communicable disease and animal bite investigations, Tuberculosis treatment, HIV and STD testing, vaccinations, and health information and referrals.

Offers Immunizations, HIV Testing and Counseling, and Tuberculosis Testing and Treatment.

Provides integrated Behavioral Health services, medication for Opioid Use Disorder program, Community Health Worker Outreach team, case management, nurse care team, telepsychiatry for medical patients, and Behavioral Health case management.

Offers public health and clinic services including family planning, immunizations, STD testing and treatment, lead abatement, emergency preparedness, maternal and child health, limited dental care, in-home asthma trigger assessments, injury prevention, and chronic disease education.

Monitors all active cases of Tuberculosis (TB) and their contacts to ensure appropriate treatment/follow-up. Monitors all clients with latent TB infections for appropriate treatment/follow-up. Collects data for statewide, CDC mandated surveilllance system.

Offers the following services: - Immunizations (all ages), offered by appointment Thursday, 9:00-11:30am & 1:00-2:30 pm. Immunization record must be brought along. Immunizations for measles, rubella, and mumps can be given between ages 12-15 months, others usually 2 months. The following immunizations are offered: DTP (diphtheria, tetanus, pertussis) IPV (Inactivated Polio Vaccine) Prevnar (Pneumocial vaccine for infants) MMR (measles, mumps, rubella) Hepatitis B-Hepatitis A Gardasil (Vaccine used to prevent cervical cancer and precancerous cervical lesions) HIB (hemophilus influenza) DT Booster or TD (for older children) Varicella (chickenpox) TDYS - Tuberculosis Clinics provide confidential diagnosis, treatment (including medication), and follow-up. (Will do a free testing for persons who have been exposed to the germ). TB clinic by appointment - HIV/AIDS antibody testing and pre- and post-test counseling. Free, confidential, and anonymous.

Provides Mantoux tuberculin skin testing (PPD screening). Medical evaluation, diagnostic testing, and follow-up treatment (if needed) are available for those with a positive tuberculosis screening test.

Offer employment physicals, driver's license physicals, sports physicals, TB testing and urine drug screenings(available per contract with employer).
