Provides locations for nursing home facilities.
Provides information, referral, and assistance relating to services and benefits for older adults such as: assessing, developing, and maintaining care plans, assisting in the completion of forms including PACE/PACENET (programs to help with the cost of medicine & prescriptions); helping to understand Medicare and Medicaid benefits; info on Senior Centers and Day Programs; transportation services; reporting when there are abuse concerns; and In-Home services such as personal care, respite care, chores and/or home delivered meals.

Provides support and services to enable older adults to continue to live in their homes and communities. Services may include Care Management, Assessment, Protective Services, Information and Assistance, PA MEDI (Insurance Counseling), Health and Wellness Programs, adult day services, in home meals, personal emergency response system, personal care, transportation, and caregiver support. The agency also has 5 Senior Centers throughout the County in which participants can enjoy a Congregate Meal and Programming.
