Program provides medication, support and education to patients diagnosed with chronic diseases. Each patient will receive an individualized care plan, including regular contact with medical professionals, community resources and strategies for self-management.

Coordinates all chronic disease programs sponsored and facilitated in partnership with other agencies; acts as a team leader for Injury Prevention, Tobacco, Oral Health, Asthma, Arthritis, Osteoporosis, as well as Cardiovascular programs. Chronic disease team nurses provide information to the public by presenting educational programs and are available for individual consultation.

Helps patients live healthy, balanced lives. Provides counseling services and resources to guide patientsâ lifestyle decisions and offer additional specialized support such as nutrition and diabetes education programs.

Offers an 8-week, Zoom-based, group educational program for adults who would like to learn how to better manage their epilepsy. Designed to increase the medical, psychosocial, and community adjustment of adults with active seizure conditions. Each week, participants will explore different topics and skills to manage their condition effectively.

Offers an 8-week, Zoom-based, one-on-one memory and cognition coaching program designed to help navigate and cope with memory challenges. Each week, participants will explore skills to compensate for memory loss and techniques to manage stress.

Provides telephone support, answers emails, and responds to online form submissions. Offers support topics such as current epilepsy treatments and alternative therapies, medication questions, support groups, seizure first aid and safety devices, employment and discrimination, emotional support, Epilepsy Foundation local offices, and Sudden Unexpected Death in Epilepsy (SUDEP).

Provides trauma-informed support and counseling for individuals with chronic illness and substance use disorder. Uses a variety of activities to promote healing and support wellness, including support groups, dance, meditation, pilates, poetry, reiki, tai chi, breathwork, and yoga.

Works to improve the quality of life of Allegheny County residents by addressing root causes of chronic diseases to prevent, detect, delay, and/or manage chronic health conditions such as heart disease, Alzheimer's disease and related dementia, diabetes, and cancer.
