Provides an array of long-term care choices based on assessment by Care Managers and establishment of a care plan with the consumer and his or her family. The Care Manager works with the family to cooperatively determine the different types of services, such as Personal Emergency Response System buttons (PERS), Personal Care, Adult Daily Living Services, and other services to help consumers stay in their own homes and communities.

Completes assessment to determine what level of care (i.e. nursing home, personal care home or community services) is appropriate for individuals on Medical Assistance to pay for a nursing home stay.

With an older adult looking for placement, they can assist older adults who need to enter a nursing home or personal care facility with determining the type of residential placement that will best meet their needs, and with securing such placement. They can also assist those in nursing facilities with returning home with supportive services.

Our health promotion programs regularly include: exercise, managing chronic conditions, health assessments. Opportunities vary but include activities such as Tai Chi, line dancing, yoga, and classes on healthy aging including good nutrition , injury prevention, emotional well being, art and music. Classes are designed to be fun and interactive. Operates as a cooling center during extreme heat emergencies (Code Red).
