Provides all-inclusive medical care and supportive services for individuals who are age 55 years and older to help them to remain in their homes and communities.
Provides all-inclusive program of medical and social services to empower older adults to remain at home, while preserving their dignity, independence, and quality of life.
Access to doctor visits, emergency visits, labs, x-rays, and other testing are eligible for discounts. Serves residents in four counties: Adams, Lancaster, Lebanon and York. HealthyCare Card (HCC) is a program of Healthy Community Network (HCN). HCN is a collaboration with Health care providers and social service agencies who are committed to working together to improve access to healthcare services for those in our region who are uninsured or under-insured. A person must enroll in health insurance if eligible for it. (For example: Medicare, Medicaid or commercial insurance through a job. A completed application with proof of income and assets will determine what discounts a person may be eligible to receive. A person with the HCC and no other prescription coverage will be given a list of pharmacies where prescriptions can be dispensed at a discount. The discount on other care will depend on

The mission of York Cancer Center is to provide excellent clinical care and comprehensive support services to cancer patients. Radiation therapy, medical oncology, surgical services, and outpatient infusion services, as well as oncology social workers, oncology dietitians, nurse educators, nurse navigators and a financial counselor are available at this facility. Also provides assistance with Dollar Energy applications. Undesignated assistance may be used for household disaster relief when funds are available. Does NOT help with security deposit assistance or other fees such as late fees etc.

Assists with life-sustaining prescriptions for people who have no resources to cover such needs. This service cannot be used on a regular basis. Clients are referred to medical assistance for possible eligibility.

* Coordinated Entry Access Site, connects individuals to programs within the Conference or refers out to community resources. Also helps qualifying low-income individuals and families with rental assistance, goal setting and budgeting. Provides bus passes for interviews or for first week of employment. Toiletries/personal grooming items provided as available. * Various rental assistance programs * Homeless Supportive Services: Provides intensive case management and referrals to other support services for single, adult, chronically homeless individuals. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. * Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.
