HCN is a collaboration with Health care providers and social service agencies who are committed to working together to improve access to healthcare services for those in our region who are uninsured or under-insured. A person must enroll in health insurance if eligible for it. (For example: Medicare, Medicaid or commercial insurance through a job. A completed application with proof of income and assets will determine what discounts a person may be eligible to receive. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. * Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.

Supplies health kits, blankets, and sweatshirts to newly arrived workers; arranges churches to visit work camps; meets emergency needs such as transportation, translation, and help with prescriptions; and leads Bible studies and worship services.

Partners with Single Care (formerly FamilyWize) to increase the health and financial stability of families in community. Through their prescription savings card, Single Care works to make prescription medications more affordable for all families, so that no one ever has to make the hard choice between the medications they need, and other basics like food or rent.

Assists with the following bills: medical (physician verified), prescriptions (must receive fax from doctor's office verifying prescription), electric (no CAP recipients), gas (no CAP recipients), water, local phone bill payment/connection (up to $20), and bus passes/gas vouchers (employment or medical purposes).
