Catholic Health Care Services (CHCS) provides short and long term nursing care at six skilled nursing homes, two independent living facilities and three assisted living facilities (personal care) throughout the region. CHCS further supports and operates various community benefit programs and oversees a treatment center for Catholic religious and priests.

Provides a continuous and ongoing array of health and/or rehabilitation services for consumers with other related conditions including individuals with paraplegia or quadriplegia (due to spinal cord injuries), spina bifida, cerebral palsy, epilepsy, seizure disorder, autism, Tourette's Syndrome, meningitis, encephalitis, hydrocephalus, multiple sclerosis, polio and anoxic brain damage. The program allows consumers to participate in community activities of their choice while maintaining the necessary skills and abilities to function as independently and productively as possible.

Offers long term care to individuals no longer able to live at home. Services include diabetes management, IV therapy, neuromuscular care, nutritional management, pain management, restorative nursing, stroke care, tracheotomy care, wound care, post-hospital and post-surgical care, medication administration, individualized care plans, and restorative care.

Offers residential living, assisted living, memory care support, skilled nursing, and rehabilitation services. Provides a variety of social, recreational, and educational programs.

Offers a continuing care retirement community striving to enhance the quality of life for those they serve in a Christ like manner. Pleasant View also has a pool, spa, steam room and fitness center.

Nursing facility Social Workers assist residents and their families with a treatment and plan of care, along with discharge planning, and facilitating details of a residents return home or transfer to another facility when such movements are appropriate.

Offers rehabilitation services, long-term nursing care, transitional care, respite, and hospice services.

Offers intermediate and skilled nursing care for persons with physical disabilities. A 99-bed facility, the Raker Center is open to residents 12 years old or older with primary diagnoses including, but not limited to, cerebral palsy, muscular dystrophy, quadriplegia, or Multiple Sclerosis. Services include daily occupational therapy activities; pastoral care including linkages with outside congregations, weekly ecumenical worship, and religious education program; physical therapy services designed to improve physical function; speech and language therapy; and recreational therapy designed to provide residents with a well-rounded lifestyle.

Nursing facility that offers physical and occupational therapy and speech pathology. Upon the physician's order, one of the rehabilitation therapists evaluates the resident and establishes a complete treatment plan.
