Wellness Center offers community health education programs including nutrition and weight management, diabetes. CPR and first aid, tobacco cessation, senior exercise programs and community health outreach.

Helps people of all ages who have juvenile or adult diabetes understand and manage their illness.

Provides primary family medical care for all ages. Services include women's health, pregnancy care, chronic disease management, nutrition services, urgent care, LGBTQ+ community care, immunization clinics, health counseling, education, guidance, and referrals for speciality medical care.

Provides licensed nurse, assistance with activities of daily living (dressing, bathing, grooming.), wellness programs, assistance with prescribed medications, diabetic management, private or shared accommodations, three meals daily, plus snacks, planned social, educational and recreational activities, transportation for scheduled activity outings, assistance with Veterans Benefits (VA) application, for those eligible, access to on-site salon services (fee for services), linen and personal laundry, housekeeping, cable services and all utilities.

Offers assistance in accessing various resources through our Social Worker, Financial Counselor and Outpatient Care Manager. In addition, they have a Certified Spanish Interpreter on site to help facilitate optimal communication with patients, care and management of diabetes and other chronic illnesses, including transitioning from hospital to home through specialized office visits.

Provides school nurses, educators and volunteers with lesson plans and classroom activities that stress the importance of eating healthy and the value of physical fitness.

In addition, our services include blood tests and lab studies; women's health care, including Pap smears; care and management of diabetes, asthma and high blood pressure.
