Home care nurses help patients achieve medical independence in the comfort of their own homes. A nurse makes visits to assess, teach and monitor patient conditions, and then communicates this information back to the patient's physician as needed. Some of the services provided by our nursing staff include: Skilled observation and assessment,therapy, Blood and specimen collection, Health education, Wound care, Other nursing services as ordered by a physician, Telehealth Home Monitoring.

Provides healthcare services and support to patients facing serious illness. Helps to manage pain and symptoms, continue activities of daily living, understand medical conditions, and explore options regarding medical and social well-being.

Provides Home health and hospice care.

Provides a comprehensive program of geriatric assessments, mental health evaluations, education, living skills, and rehabilitation that identifies and treats older adults who are at risk for depression, dementia, fall risk, and incontinence.

Assists individuals with serious mental illness who are residing in a community setting and are experiencing difficulties maintaining adherence to their medication regiment.

Provides in-home medical care and non-medical care for individuals to gain, maintain, and improve skills in the areas of self-care, daily living activities, social skills, and to enable the person's involvement in community activities.

Provides personal care services, to include meal preparation, light housekeeping, grooming and dressing assistance, errands and other personal care services.

Provides Skilled Nursing Care, Palliative Care, Medical Social Workers, Therapy Services and Home Health Care.

Provides several hospice and palliative care services including pain management, assistance with personal care, psycho-social needs, complementary therapies, assistance with activities of daily living, nursing care, bereavement and grief counseling services, and volunteer help.

Specially trained Registered Nurses work in the home with high-risk newborns or children with serious illnesses. Teams of professionals to provide parents with education and support to care for their children.

Provides Heart Failure Disease Management, Cancer Care Specialty Services, Orthopedic Rehabilitation, Recovery from Fractures, Total Hip and Knee Replacement Surgery, PT/INR IV/Infusion Therapy, Wound Care, Reconditioning after an Illness, Post Surgical Patients, Cardiac Rehabilitation, Stroke Rehabilitation, Diabetic Care, Pulmonary Care, Homemaker Service, and Respite Care.

Abington Jefferson Health's Kind Pediatric Hospice program is committed to providing compassionate and comprehensive care to children and their families for serious illness through the end of life. Our inpatient unit, which offers specially designed pediatric rooms and a dedicated family lounge, is devoted to children from newborns to young adults. The staff provides care for children in the advanced stages of illness, including cancer, congenital conditions, neurological diseases, genetic conditions, HIV/AIDS and traumatic injuries. Abington Health also offers in-home pediatric hospice services.

A free program especially for women who are pregnant with their first baby. Participants can enroll early on in their pregnancy up to the 28th week. A specialized maternal/child nurse visits clients at home during pregnancy and may continue until the baby turns two years old, providing education and support in childbirth and childcare.

Home Health Care

Offers private duty skilled home health care and treatments for chronic and acute illnesses and disabilities for adult, elderly and pediatric patients. Provides unskilled in home care aides and other services to assist with bathing, daily hygiene, meals, mobility and transfers. Includes personal care, companion care, and respite care.

Offers care aides, companions, or nurses in the clients home.

Offers a continuum of healthcare, including post-operative care, respiratory services, infusion therapies, disease specific therapies, respite care, nursing facilities, home nursing, and hospice.

Provides non-medical personal assistance home care, home health nursing care and skilled physical, occupational and speech therapist. Services are provided in the individuals home or other independent living environments. Accommodates ongoing care, one-time-only requests when regular caregiver/family member is out of town.

Provides part-time, skilled, intermittent home health services that have been ordered by a physician and are medically necessary to the treatment of an illness or injury. Provides care for people of all ages-from infants to children and adults-recovering from an injury or recuperating from an illness. All services are provided in the comfort and privacy of the patient's own home.
