Pennsylvania Assistive Technology Foundation - Community HealthChoices

Provides a waiver for individuals with a disability who previously received services through the Independence, CommCare, or Aging Waivers. These individuals select or are automatically enrolled in one of three managed care organizations (MCOs) that have contracts with the state. Services are obtained through service providers who have contracts with these managed care organizations. Services include all the Home and Community-Based Services formerly provided by other waivers, plus a few new ones, for older adults and adults with physical disabilities or traumatic brain injury.

Roads to Freedom Center for Independent Living of North Central PA - Home Care

Roads to Freedom Home Care Service enables adults with disabilities, who are nursing facility eligible, to remain in their own homes and continue to live with support and services. Services offered include assistance with bathing, dressing, transferring, medication reminders, shopping, transportation, or other activities specific to the individual's plan of care. Roads to Freedom Home Care accepts Community Health Choices, OBRA waiver, Act 150 program, and private pay.
