Roads to Freedom Home Care Service enables adults with disabilities, who are nursing facility eligible, to remain in their own homes and continue to live with support and services. Services offered include assistance with bathing, dressing, transferring, medication reminders, shopping, transportation, or other activities specific to the individual's plan of care. Roads to Freedom Home Care accepts Community Health Choices, OBRA waiver, Act 150 program, and private pay.

Works to reduce caregiver stress. The emphasis is to respond directly to the needs of caregivers who are caring for a functionally dependent elderly consumer. Financial reimbursement may be provided for out-of-pocket caregiving expenses, such as personal care services, supplies, assistive devices, or home modifications. Total household income determines the specific percentage of monthly reimbursement.

Waiver recipients are provided care in the community; appropriate for those who would otherwise be eligible for nursing home care. Applicants will be assessed to check if sufficient formal and informal services and supports can be developed and implemented so as to safely and adequately meet the person's needs.

Provides person-centered case management services to individuals enrolled in Pennsylvania’s Community Health Choices (CHC) program and other state waiver programs.

Develops and implements creative alternatives to placing Medicaid-eligible individuals in medical facilities such as nursing homes and Community Residential Facilities (CRF). Recognize that many individuals who are at risk of being placed in a medical facility can be cared for in their natural homes and communities through the use of home and community-based waiver programs.
