Provides Early Intervention Service (EIS) coordination for Allegheny County families. Connects children in need with intervention services provided at their home or day care location.

Offers services for individuals with substance use disorders. Provides screening and level of care assessment for substance use disorder, referral to treatment, and support until admission if space is not available in an appropriate setting immediately. Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.

Provides case management services to individuals with mental health needs in Cambria County.

Provides public health education, preventive nursing services, emergency preparedness, immunizations, communicable disease surveillance and investigation, HIV testing and counseling, community empowerment, and other nursing services as mandated by PA legislation. Provides adult and child immunizations, HIV/AIDS referrals, and health resources.

Provides free assessment, evaluation referrals and follow-up case management services to substance use programs and services.

Provides services for preventing homelessness, and improving stability for veterans.

Case management services for adults with behavioral health issues identified as serious mental illness. Case managers develop recovery plans for individual clients, arrange for required services, monitor services and serve as an advocate for the individual. Offers mental health support services including information/education, intervention, self management and transitional services. Also provides post disaster mental health services.

Intensive children and youth program that works with families to help them enhance their strengths in crucial areas such as child care, discipline, physical standards, budgeting, and hygiene. This program provides the necessary services to keep children in their homes.

Provides food, financial assistance, budget counseling, and case management and referral services.

Provides services at an intensive outpatient and outpatient level of care. Offers individual, group, and case management services. Provides childcare and transportation to and from the Consortium. In addition, a Family Service Coordinator provides support and resources for the consumer's children.

Meets with each resident and conducts a social history and assessment upon admission. When short-term residents are ready to return home, social services staff helps residents determine the best options for a safe discharge. This includes reliance on home health care, durable medical equipment, community services, and support groups.

Provides long-term case management services to individuals and families who need assistance securing and maintaining basic needs (housing, food, utilities, medical, etc.), navigating the human services system for appropriate resources, and following client-centered goals and service plans to improve overall self-sufficiency.

Provides emotional support, advocacy, education, budget counseling, and goal-setting for those calling in. Case managers also help individuals navigate the resources and funding available in Centre County.

Program that develops plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual.

Offers a support service to assist Westmoreland County residents who are currently involved in the criminal justice system and living with behavioral health concerns (with or without substance use disorder). Criminal Justice Liaison collaborates with providers, court-related personnel, and others to help coordinate treatment, care, service planning, and connect to higher level case management services, as needed.

Provides case management services tailored to client's needs and goals. Holds meetings at locations convenient for the client. At first meeting, case managers work with clients to set goals they want to achieve with FOF's help. Case managers contact clients at least every other week and will meet in-person at least once a month for up to six months. Clients can choose to stop participating in the program at any time. Provides financial and relationship counseling through partner agencies.

Provides mental and behavioral case management services that include supports coordination, information and referral, consultation and education, counseling, therapy, and family driven support services.

Provides those who are HIV positive with assistance in obtaining medical care, counseling, mental health treatment, substance abuse treatment, food and housing. Also provides prevention and education programs to high risk individuals and groups. Services may be provided at a client's home or by phone.
