Provides reentry supportive services for families impacted by the criminal justice system. Offers services that include housing assistance, mentoring, case management, referral services, food, clothing, educational support, employment assistance, photo identification cards, driver's licenses, and bus passes for employment.

Intensive children and youth program that works in specialized case management teams to provide and work closely with parents and caretakers of children in placements to return the children to their homes as quickly as possible. The family is offered therapeutic programs, FGDM (Family Group Decision Making) and any other services to aid in the reunification.

Aims to restore relationships and create healthy families. Services include outpatient therapy, play therapy, parent coaching, group therapy, and family therapy. This service is also available to families working toward reunification.

Provides reentry services that include housing, mentoring (groups, families, one-on-one), family reunification, case management, housing for battered women and children, referrals, and services for at-risk youth. Also provides an annual scholarship to local seniors through the Julian Mallory Scholarship Fund.

The primary goal of the program is to implement brief, intensive, family-focused reunification service for families who have children placed in foster or kinship care in Luzerne County. The program is designed to reduce the amount of time children spend in out-of-home placement by strengthening families, building resources, addressing parenting concerns, and creating a plan for reunification. The team builds a child-focused partnership between the family, the community and human service agencies in order to support the family and facilitate planning for the safe reunification of children with their birth parents. Casemanagement services are offered, as well as parenting education, anger/stress management, family enrichment, and resource coordination.

Provides foster care and the recruitment and training of foster parents, adoption, parent support, independent living, and shelter care.

Provides in-home, diversion, placement transition, and reunification services for children and their families in Lehigh and Northampton Counties.

Identifies families with children in foster placement whose prospects for successful reunification are good, provide supportive and all other services needed to attempt and facilitate reunification.

Provides permanency services to children who have entered out-of-home care. The program uses an in-home, family focused approach for children who have been separated from their families due to abuse, neglect, dependency, or delinquency issues. Staff work directly with family members through case management and counseling to resolve the issues leading up to the family’s separation and to ensure the child can return home safely or find permanency through other family members, foster parents, or adoption.

Provides intensive services in the home, working in cooperation with the family and Children & Youth Services to determine risk factors. Helps the family develop goals that address those risk factors so that children can be returned to, or remain in, their homes.

Provides intensive interventions blending case management and therapeutic services, educational services, and out-client services. Offers Family counseling, parent counseling, and educational services (parenting, job search, housekeeping, etc.)

Reunites children who are in foster care with their parents. Connects families with other community resources that will support them in preventing re-placement of their children in care and in maintaining long-term stability.

Intensive in-home services for child at risk. Service can be provided by in house or purchased programming. Referrals must be made by Children and Youth.

Provides a caseworker to families that have had children removed from the home to safely reduce barriers that prevent reunification of the family unit.

Families who would benefit from further supportive services work with a caseworker to develop a Family Service Plan. The goal of this service is to work with families to prevent out-of-home placements and/or help to reunify families through short-term intensive in-home services. Family Counseling works closely with the caseworker or juvenile probation officer to strive toward family success.

Family Finding Service aims to discover relatives and engage them in the planning and decision making of important aspects of child's life. Identifies relatives to multiple degrees and other supportive adults, estranged from or unknown to the child, in hopes of identifying willing relatives to become permanent connections
