Operates the Children's Home of York's Permanency Program. Provides a variety of services for children and their biological, foster, and/or adoptive families from birth through the age of 18. Maintains an ongoing process of recruitment, screening, and training which enables us to offer diverse geographical and racial/ethnic environments essential to the successful placement of children and sibling groups of all ages who have a variety of emotional and psychological needs.

Intensive in-home services for child at risk. Service can be provided by in house or purchased programming. Referrals must be made by Children and Youth.

Provides reentry supportive services for families impacted by the criminal justice system. Offers services that include housing assistance, mentoring, case management, referral services, food, clothing, educational support, employment assistance, photo identification cards, driver's licenses, and bus passes for employment.

Offers community parenting classes, fatherhood basics and activities, Family Reunification Program, Family Group Decision Making, Parents as Teachers Home Visiting, Juvenile Probation Workshops and Budget Workshops.

Identifies families with children in foster placement whose prospects for successful reunification are good, then provides supportive and all other services needed to attempt and facilitate reunification.

Provides in-home parenting support, parenting classes, supervised visitation, and reunification support for up to 90 days.

Intensive children and youth program that works in specialized case management teams to provide and work closely with parents and caretakers of children in placements to return the children to their homes as quickly as possible. The family is offered therapeutic programs, FGDM (Family Group Decision Making) and any other services to aid in the reunification.

Family Finding Service aims to discover relatives and engage them in the planning and decision making of important aspects of child's life. Identifies relatives to multiple degrees and other supportive adults, estranged from or unknown to the child, in hopes of identifying willing relatives to become permanent connections

Provides in-home, diversion, placement transition, and reunification services for children and their families in Lehigh and Northampton Counties.

The primary goal of the program is to implement brief, intensive, family-focused reunification service for families who have children placed in foster or kinship care in Luzerne County. The program is designed to reduce the amount of time children spend in out-of-home placement by strengthening families, building resources, addressing parenting concerns, and creating a plan for reunification. The team builds a child-focused partnership between the family, the community and human service agencies in order to support the family and facilitate planning for the safe reunification of children with their birth parents. Casemanagement services are offered, as well as parenting education, anger/stress management, family enrichment, and resource coordination.

Offers family, couples, and individual counseling. Also provides anger management, art and play therapy for children, and parenting training in a group therapy format.

Provides comprehensive and intensive child placement services that include clinical and case management activities. The ultimate goal of the program is to minimize the length of stay in the foster care system and work towards permanency (reunification with parents and/or family members, adoption, or transition to independent living).

Provides reentry supportive services for families impacted by the criminal justice system. Provides intake services division, family preservation services division, reunification and permanency services division, family group decision making, and other services.

Provides intensive interventions blending case management and therapeutic services, educational services, and out-client services. Offers Family counseling, parent counseling, and educational services (parenting, job search, housekeeping, etc.)

Provides reentry and individual/family services, including outpatient substance use disorder treatment, mentoring, anger management intervention, cognitive behavioral treatment, employment training, parenting skills classes, and transitional housing.

Families who would benefit from further supportive services work with a caseworker to develop a Family Service Plan. The goal of this service is to work with families to prevent out-of-home placements and/or help to reunify families through short-term intensive in-home services. Family Counseling works closely with the caseworker or juvenile probation officer to strive toward family success.

Provides intensive services in the home, working in cooperation with the family and Children & Youth Services to determine risk factors. Helps the family develop goals that address those risk factors so that children can be returned to, or remain in, their homes.

Provides permanency services to children who have entered out-of-home care. The program uses an in-home, family focused approach for children who have been separated from their families due to abuse, neglect, dependency, or delinquency issues. Staff work directly with family members through case management and counseling to resolve the issues leading up to the family’s separation and to ensure the child can return home safely or find permanency through other family members, foster parents, or adoption.

Reunites children who are in foster care with their parents. Connects families with other community resources that will support them in preventing re-placement of their children in care and in maintaining long-term stability.

Provides in-home parenting support, parenting classes, supervised visitation, and reunification support for up to 90 days.

Provides reentry services that include housing, mentoring (groups, families, one-on-one), family reunification, case management, housing for battered women and children, referrals, and services for at-risk youth. Also provides an annual scholarship to local seniors through the Julian Mallory Scholarship Fund.

Provides foster care and the recruitment and training of foster parents, adoption, parent support, independent living, and shelter care.
