Areas addressed may include socialization and decision-making skills, basic banking and budgeting, meal preparation, assistance with community integration, transportation as well as health and safety issues.

Facilitates relationships between individuals and the communities in which they live. Via connects people to their community through volunteer, recreational, social and educational activities. Via helps individuals discover preferences and choose recreational and volunteer activities in their community to participate in as independently as possible.

Provides residential options for individuals with developmental disabilities and/or for mental health consumers. Options for persons with developmental disabilities include: Community Homes, Supported Living Services, and Life Sharing. Options for Mental Health Consumers include: Personal Care Homes, Community Residential Rehabilitation (CRR), Supportive Housing: Permanent, Supervised Living, and Independent Living.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Through this arrangement, individuals with disabilities become active, participating members of another family. Reverse Lifesharing is also an option for someone who wishes to remain in their own home and have a family or person move in with them.

Provides a community home for individuals diagnosed with intellectual disabilities.

Provides living arrangements in community residences for chronically mentally ill adults and selected children. Operated by providers under contract to the agency, there are several types of programs and multiple sites. Residential services include community residential facilities (CRF), community residential rehabilitation (CRR) services, long-term structured residences (LTSR), and residential treatment facilities (RTF).
