Coordinates and carries out services designed to allow individuals with disabilties to live as independently as possible in the community. Services include Lifesharing, OBRA, Supervised Community Homes, and Home and Community Based Services.

Offers 20 one-bedroom units of safe, affordable and well maintained housing for eligible clients.

Provides supports for adults with disabilities to live in the comfort of a JEVS community home and maintain their independence. Services include shopping, cleaning and grooming; financial counseling and money management; access to medical appointments, and help and support for individuals to connect with others in their natural surroundings or to integrate into their communities and participate in activities in the community.

Provides residential services and living arrangements that offer several levels of support and encourage varying levels of independence.

Offers a variety of day and supportive living services, such as in-home and community supports, day services, respite, companion services, community participation supports, supportive living, supported employment, and deaf services with ASL.

Provides an opportunity to become self-sufficient and have many opportunities to grow and explore their interests. Each person receives supports within their own home or apartment or in a home or apartment rented by Keystone.

Provides supports to individuals with mental health or intellectual/developmental disability diagnoses who live in their own residences or are in transition to living in the community. Promotes self-sufficiency and individuality, and maximizes the individuals potential for independence. Each participant is active in the planning and implementation of their supported living goals.

Services for adults with intellectual and developmental disabilities and medically complex conditions include: A) Residential services including Lifesharing and Community Living B) Supported Living C) Day programs D) Relief Support E) Hourly Support Services

Offers a full range of therapeutic, home and community-based services for people with intellectual and developmental disabilities. Services include behavioral supports, community integration activities, community residential services, community residential living, counseling, home and community habilitation, Life Sharing (Family Living), specialized services for autism, supported employment, and supported living services.

Provides community residential services, pre-vocational training services, and intermediate care for adults with intellectual disabilities.

Provides living arrangements in community residences for chronically mentally ill adults and selected children. Operated by providers under contract to the agency, there are several types of programs and multiple sites. Residential services include community residential facilities (CRF), community residential rehabilitation (CRR) services, long-term structured residences (LTSR), and residential treatment facilities (RTF).

Helps coordinate services including personal care assistance, home modifications, transportation, and discharge planning. Also administers the OBRA and Independence, and CommCare Waiver Programs.

Offers supported living in the community and and 24-hour supervised residential living in licensed community homes for individuals with intellectual and developmental disabilities.

Provides community-based residential support to adults with intellectual and development disabilities.

Services are provided for functionally disabled persons with medical problems who have been determined to be nursing home appropriate but desire alternative care in the community.

Individuals with a disability live and share a home and life experiences with supportive persons, to whom they are not related, in a family environment. Through this arrangement, individuals with disabilities become active, participating members of another family. Reverse Lifesharing is also an option for someone who wishes to remain in their own home and have a family or person move in with them.

Provides in-home and community caregivers to support people of any age with physical, intellectual and/or developmental disabilities. Caregiver supports can be provided for a few hours a month, daily, weekly and/or 24-hour bases. Builds independence, as well as ensures the health and safety of the individuals that are supported.

Assist individuals who currently reside in nursing facilities to transition back into home and community based settings.

Connects individuals with the independent living services needed to achieve independent living.

Services are provided for functionally disabled persons with medical problems who have been determined to be nursing home appropriate but desire alternative care in the community.

Offers assistance with mail reading, check writing, daily living skills, life skills education, transitions assistance, medical transportation and vision evaluations.

Licensed Community Homes are able to provide round the clock assistance and supervision of an individual both in the home and in the multitude of social and community gatherings that are offered through providers. Life Sharing through Family Living allows an individual to live in a family environment with trained individuals providing care. Depending upon the consumer, these homes can be licensed or unlicensed.

Provides adult day programs and training, supportive employment services, children’s services-three approved private schools, medical rehabilitation, camp and respite programs, and residential and veterans’ services. The essential tools and resources provided enable individuals with disabilities to live their best life.

Provides residential options for individuals with developmental disabilities and/or for mental health consumers. Options for persons with developmental disabilities include: Community Homes, Supported Living Services, and Life Sharing. Options for Mental Health Consumers include: Personal Care Homes, Community Residential Rehabilitation (CRR), Supportive Housing: Permanent, Supervised Living, and Independent Living.
