A separate, intensive/blended case management unit provides continuity of care for those individuals with serious mental illness residing in Lehigh and Northampton counties who no longer require the intensive level of multidisciplinary assertive community treatment team services. The Lehigh Valley ACT Permanent Supportive Housing program provides independent, scattered site housing funded through a McKinney-Vento grant to chronically homeless individuals with serious and persistent mental illness.
