The FACT Team is specifically designed for those clients meeting the several criteria for ACT but have a forensic criminal justice history. Services include psychiatric evaluation, medication monitoring, service coordination, psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, leisure time training, service coordination, crisis assessment, intervention symptom assessment/management, medication administration/monitoring, and Other Major Supports.

A program for Berks County Medical Assistant recipients, the program is based on the ACT approach which was developed to serve individuals who have the most serious and intractable symptoms of severe mental illness who consequently have the greatest difficulty with basic daily activities.

Trans-disciplinary team approach that provide acute, active and ongoing community-based clinical services, psychiatric treatment, assertive outreach, rehabilitation and support. Provides psychosocial services directed to adults with mental illness who often have co-occurring problems such as substance abuse or are homeless or involved with the judicial system. Services are comprehensive and highly individualized. Services vary in intensity based on the needs of the person served. Services include:

Provides a voluntary community based program offering comprehensive mental health, addiction assessment, rehabilitation, and support services. The majority of the assertive outreach approach occurs in the consumer's community through a trans-disciplinary team which includes a psychiatrist, registered nurses, mental health professionals, and specialists in the fields of chemical dependency, vocational rehabilitation, housing, and peer support.

Provides a recovery-oriented service for persons with serious mental illness who have typically not benefited from more traditional outpatient services. Health professionals help clients to stabilize and educate about medication; to support and strengthen interpersonal relationships; to find and maintain meaningful employment; and to develop day-to-day skills such as money management, housekeeping, grocery shopping, meal preparation, laundry, using public transportation, and utilizing community-based services such as dentists and PCP's.
