Provides Outpatient Mental Health Services including Psychiatric Evaluations, Psychiatric Medication Management, Individual and Family Psychotherapy for adults and children for a range of relationship issues, mental health and emotional health issues.

Provides an intermediate level of case management to support people with serious mental illness. Offers a Southeast Asian element to service languages of Southeast Asia.

Provides case management, day programming, site based psych rehab (group rehab), emergent funds (depends on need), crisis intervention, delegate services, outpatient funding for counseling and psychiatry if an individual has no insurance, and a housing program. For MR or IDD consumers we offer emergent funds, supports coordination, day programming, group home placing/assistance, crisis intervention and delegate services. - Provides early intervention for children from the age of 0 months to 2 years and 9 months of age. This service is provided for any child that may be displaying a delay physically, verbally, speech development, etc.

Provides treatment to young adults who recently experienced their first episode of psychosis. Services include low-dose medication management, individual and group therapy, family psychoeducation and support, case management, supported education and employment, and peer support services. Services and treatment are offered for up to 2 years.

Provides case managers who help individuals diagnosed with serious mental illness access community services needed in order to live independently and maintain mental and physical health. Case managers assist in areas such as, but not limited to: mental and physical health care, housing, education/work, daily living, income/benefits, and drug/alcohol services.

Assists clients in identifying, facilitating, and coordinating available services, and educating clients on available resources and how to access them. Provides assistance in all areas of the individual’s life, including housing, physical and mental health care, education, employment, and recreation. Also acts as a liaison between the client’s service providers.

Provides Blended/Intensive Case management and Resource Coordination. Case managers serve children and adults who have a serious and persistent mental illness which interferes with their capacity to function in significant areas of daily life over a prolonged period of time. Case managers provide consumers with information about appropriate community services and resources and support them in accessing these services. Services include psychiatric evaluation, medication monitoring, service Coordination, psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, and leisure time training.

Provides non-clinical primary mental health services to insure that the most seriously mentally ill individuals receive additional support geared to improving the quality of their lives and reducing psychiatric hospitalization. Casemangers use natural occurring supports in the community, i.e., family, friends and neighbors. Services include: Psychiatric Evaluation, Medication Monitoring Service Coordination, Psychiatrist Supports Crisis Intervention, Social Skills Training Psychiatric Rehabilitation, Vocational Assessment Housing Supports, Activities of Daily Living Peer Support Services, Leisure Time Training

Day Hospital/Intensive Outpatient mental health treatment program for those who have a diagnosis of mental health illness and who are experiencing serious to severe levels of emotional distress. Program is designed to allow individuals to remain with their families and community. Program varies from 3-6 hours and includes the following services: * psychiatric evaluation * individual therapy * group therapy * psych-education regarding symptom management, coping skills development and problem solving * Psychopharmacological consultation * Treatment includes the participation of family members and other significant persons * Individual meetings to review treatment * Discharge planning, including aftercare programs. * Children and adolescents receive age appropriate education services coordinated with students home school district

An assessment Unit provides or connects clients with appropriate services. The case management staff provides ongoing support to the individual and the family. Contracted provider agencies offer a variety of general and intensive services in the community. 24-hour crisis intervention also available.

Provides intensive therapeutic interventions to help youths with serious emotional health challenges stay with their families and to minimize the need for placement in more restrictive environments such as inpatient or residential settings. The program promotes stabilization of mental health symptoms, provides families the support needed to re-stabilize their home environment. Treatment is done in the home, school or community.

Case managers assist individuals obtain supports that help reduce hospitalization so that they can continue to live in the community in the least restrictive setting possible. The Extended Assessment Program offers up to 24 hours of assessment and case management to children in Delaware County. The program links children with the services that will best aid their overall growth and development in school, at home, and within their community

Mental Health Case Management focuses on community integration and the life domains of housing, financial benefits, education, employment, physical and mental health, and natural social supports. There are two levels of support: A) Administrative Case Management: Primarily office-based and less intensive; available to children and adults of Bucks and Montgomery Counties B) Blended Case Management: Primarily community-based and offers a more intensive level of service; available to children and adults of Bucks and Montgomery counties; recovery coaching is a model of Blended Case Management for adults that offers team-delivered services with a clinical consultation component

Assists with the delivery of Behavioral Health Services for children and their families.

Acts as the entry point to mental health services in Washington County. Services include, but are not limited to the following: Acute Inpatient Behavioral Health, Outpatient and Partial Hospitalization Treatment, Mobile Medication, Assertive Community Treatment Teams, Supported Employment, and a variety of other recovery-oriented services such as Psychiatric Rehabilitation, and Clubhouse and Peer services. Multiple residential and housing options are available as well for those who meet the criteria.

Intensive Case Management (ICM) & Resource Coordination Case Management (RCCM) provides consumer specific service plans that are goal and outcome oriented. The program is designed to insure access to needed resources such as medical, social, educational, vocational, and other services. It also monitors service delivery, assessment and planning, networking and advocacy.

Community-based intensive case management for children with mental health issues.

Offers individual therapy, medication evaluation, and medication management led by a licensed psychiatrist/CRNP and masters-level clinicians.

Offers inpatient treatment that is individually tailored to meet each patient’s needs. Short-term crisis stabilization and consistent care in a secure environment. Also offers inpatient hospitalization. Available services include: Diagnosis and evaluation by a board-certified psychiatrist, on-site gymnasium, music, art, movement, social work and nursing treatment groups, Family Education Group, and private dining.
