Mental Health Case Management focuses on community integration and the life domains of housing, financial benefits, education, employment, physical and mental health, and natural social supports. There are two levels of support: A) Administrative Case Management: Primarily office-based and less intensive; available to children and adults of Bucks and Montgomery Counties B) Blended Case Management: Primarily community-based and offers a more intensive level of service; available to children and adults of Bucks and Montgomery counties; recovery coaching is a model of Blended Case Management for adults that offers team-delivered services with a clinical consultation component

Offers psychiatric, psychological, and counseling services to children and adults of all ages.

Provides Case Management Services such as Intensive Case Management (ICM), Resource Coordination (RC), and Service Case Management (SCM). These programs are designed to reduce homelessness and hospitalizations of persons suffering from severe and persistent mental illness.

Provides Blended/Intensive Case management and Resource Coordination. Case managers serve children and adults who have a serious and persistent mental illness which interferes with their capacity to function in significant areas of daily life over a prolonged period of time. Case managers provide consumers with information about appropriate community services and resources and support them in accessing these services. Services include psychiatric evaluation, medication monitoring, service Coordination, psychiatrist supports, crisis intervention, social skills training, psychiatric rehabilitation, vocational assessment, housing supports, activities of daily living, peer support services, and leisure time training.

Assists clients in identifying, facilitating, and coordinating available services, and educating clients on available resources and how to access them. Provides assistance in all areas of the individual’s life, including housing, physical and mental health care, education, employment, and recreation. Also acts as a liaison between the client’s service providers.

Assists individuals who are not insured or underinsured in seeking mental health services. A referral from the BSU is necessary to begin receiving county-funded mental health services. An assessment will be completed to ensure that clients are provided with an unbiased choice of accessible and appropriate services.

Provides case management services to adults, children, and adolescents with a mental health diagnosis. Assists clients in locating services and resources to meet their needs and monitors the service delivery.

Assists consumers to gain entry to the mental health system and coordinates other services. Offers help which includes preauthorization of services and provides referral to specialized care and other community programs. Monitors quality of service delivery, educates about assisted outpatient treatment (AOT), and provides emergency services during daytime hours.

Blended Case Management is a service that assists in gaining access to psychiatric, medical, social, educational, and other services. Case management services include linking with services, monitoring of service delivery, gaining access to services, assessment and service planning, problem resolution and help in using community resources.

Provides enhanced behavioral health treatment services to children and individuals up to age 18 who have a mental health diagnosis and/or Autism Spectrum Disorder. Offers family supportive services and counseling. Services can be provided in the child's home, community, or school.

Offers inpatient treatment that is individually tailored to meet each patient’s needs. Short-term crisis stabilization and consistent care in a secure environment. Also offers inpatient hospitalization. Available services include: Diagnosis and evaluation by a board-certified psychiatrist, on-site gymnasium, music, art, movement, social work and nursing treatment groups, Family Education Group, and private dining.

Provides information, intake, assessment, referral, authorization and monitoring of children, adult and senior citizens with mental health concerns. Clinical staff develop diversion options for individuals admitted to local inpatient behavioral health units, who are at risk for state hospitalization and actively participate in discharge planning for all Erie County residents during their treatment at Warren State Hospital. The point of entry for the enrollment of individuals in the public mental health system.

Assures appropriate services are available for youth and their family who are experiencing mental health challenges. Contracts with agencies who provide an array of services and resources which are focused around trauma informed principles, recovery, resilience, and self-determination. Services include high fidelity wraparound, family based mental health, multisystemic therapy, outpatient treatment, blended case management, and mental health respite services.

Provides mental health case management for adults with a severe psychiatric illness. Coordinates housing, finance, medical, psychiatric, vocational, educational, and recreational services to stabilize client.

Specialty Programs include: Partial Hospitalization Program (PHP) for adults, is a 3 or 5-day program based upon clinical acuity. Partial Hospitalization Program (ALPHA PHP) for children and adolescents, grades K through 12, includes academic instruction and is a year-round program. Psychiatric Rehabilitation is a voluntary, non-clinical program that helps restore community functionality for individuals with mental illness.

Assists consumers in accessing the system via the Base Service Unit (BSU), assessing financial resources, helping consumers choose appropriate services and coordinating all efforts towards service goals. The current array of case management includes administrative case management, resource coordination, and intensive case management.
