Offers a recovery community for individual who are experiencing distress and feel that they would benefit from participation in a short term, 24 hour peer-to-peer supportive environment with others who have experienced recovery and can provide support.

You will live in the community. The level of support you need will determine where you will live. You may live in a group home or in an apartment building located in Northeast Philadelphia. In our group home, you will live with 8 other individuals and share a bedroom with one person. Staff are present in the home at all times. You and your housemates will prepare and eat meals together and share in completing household chores. In the apartment building, you will share a one or two-bedroom apartment with one other person. PATH has a separate apartment in the same building where staff are available at all times to assist you when needed. You and your roommate will share in completing household chores. You can decide to purchase and prepare meals together or individually.

Provides temporary, transitional living arrangements for individuals who are registered in the public mental health system and are temporarily homeless. Participants are assessed as able to benefit from the service and have the ability to function within the structure and support of the program and comply with the program rules.

Provides short term, intensive and individualized treatment services for the identified persons in order to successfully re-integrate them into the community. Facility consist for two 3 bedroom semi-detached homes and accommodates up to seven individuals. Program length ranges from 6-12 weeks depending on individual's needs. Provides services in the homes of eligible adults who require assistance with bathing, grooming, dressing, ambulation, meal prep or supplemental housekeeping. Persons receiving this service may have a difficult time completing these activities on their own. With this service, older adult can remain independent at home. Services may include: Personal Care, Caregiver Support, Care Management, Home Delivered Meals, Protective Services.

Helps adults recovering from mental health challenges to live in the community while developing independent living skills. Offers single or shared apartments and shared single-family houses.

Together with your counselor, you will develop a rehabilitation plan. This plan will support you in being successful in the goals you choose. Your counselor and other program staff will assist you in learning skills both at the site and in the community. Community skill building opportunities might include learning how to use community resources, such as public transportation, markets, banks, cultural and educational resources. You will participate in groups that teach you about your illness and support you in managing your symptoms, making positive choices and self-directing your recovery.

Offers treatment to adults with a mental health diagnosis who need more intensive treatment than outpatient services provide. Individuals attend the program multiple days per week and receive group therapy, medication management and education on psychiatric and wellness topics.

Clients are invited to join daily recreational activities.

Provides mobile peer support services offered by certified peer specialists who have lived with and experienced mental health challenges. Services do not replace traditional therapies, but instead offer an alternative to individuals who may feel alienated and disconnected from the mental health system.

Matches volunteers in one-to-one friendships with people in mental health recovery based on age, gender, and interests.

Offers a community centered focused membership for those experiencing mental illness. Provides focus in: receptionist/clerical work, data collection and entry, in- house newsletter publication, computer skills, employment/educational goals, developing continuing education skills, cooking, meal planning, budgeting, gardening, and wellness activities.

Clubhouse is a recovery oriented program for adults designed to give members the opportunity to learn to manage their illness and develop the skills needed to achieve recovery. The program is based on a work-ordered day and provides vocational training in the areas of clerical/computers, kitchen, education, and maintenance.

Provides case and care management referrals in order to connect participants with community resources and services such as transitional and outpatient mental health services.

Day Hospital/Intensive Outpatient mental health treatment program for those who have a diagnosis of mental health illness and who are experiencing serious to severe levels of emotional distress. Program is designed to allow individuals to remain with their families and community. Program varies from 3-6 hours and includes the following services: * psychiatric evaluation * individual therapy * group therapy * psych-education regarding symptom management, coping skills development and problem solving * Psychopharmacological consultation * Treatment includes the participation of family members and other significant persons * Individual meetings to review treatment * Discharge planning, including aftercare programs. * Children and adolescents receive age appropriate education services coordinated with students home school district

Provides services specifically designed for the seriously mentally ill.

Individuals who are 18 years and older who have been diagnosed with a serious mental illness (e.g. bipolar disorder, schizophrenia). Additional criteria, call for more information.
