Provides intensive (9 hours per week) drug and alcohol treatment for adults needing a higher level of care, or for those being discharged from inpatient rehabilitation substance abuse treatment.

Offers medication-assisted treatment (MAT) and personalized outpatient treatment for adults struggling with opioid use disorder. Offers counseling, and Methadone, Suboxone, Buprenorphine, and Vivitrol maintenance programs.

You will be assigned a primary case manager.. You will meet with a case manager who will form a partnership with you to facilitate your recovery. In addition to your case manager, other team members will also be available to assist you. You will meet with a case manager at your home, at PATH, or in the community. You and a case manager will identify goals that you would like to achieve and develop a plan to achieve your goals. You will receive help from a case manager to obtain mental health, substance abuse or other medical treatment. You will receive help from a case manager to obtain services such as housing, health insurance, financial benefits and other needed resources. You will be able to contact a case manager for help whenever you experience a crisis, 24 hours a day, 7 days a week. You will receive support and encouragement from a case manager as you enter recovery and learn to become more independent.

Provides outpatient drug and alcohol treatment services with three levels of care; Partial Hospitalization Program, Intensive Outpatient and traditional outpatient.

Provides halfway house, residential, regular/intensive outpatient services. Offers substance abuse treatment and treatment for co-occurring serious mental illness and substance use disorders. Provides Intensive Outpatient treatment program recommended in some circumstances by a clinical and medical assessment; recommended for those who do not need medically-supervised detox.

Provides acute medical detoxification to chemically dependent persons as the first phase of the recovery process. A concerted effort is made to lead the patient into the next phase of a total recovery program, i.e., outpatient, residential, or 12 step programs. Complete nursing assessments are completed on all patients admitted to the department to include: Detox Unit Initial Intake Preliminary treatment plans, Physical assessment, and Psychosocial assessments.

Provides a Substance Use Outpatient Services and Substance Use Intensive Outpatient Services (IOP) for individuals with substance abuse disorder.
