Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.
