Services include: Advocacy of Individual rights Development and maintenance of an Individual Support Plan (ISP). Determining the need for services and developing "outcomes." Identifying and locating providers of services. Coordinating waiver services when funding is available. Ensuring services are received as outlined in the ISP. Ensuring health and safety.

SAM provides case management services to individuals enrolled in the early intervention, mental health and intellectual disabilities case management program.

Operates as a day program offering supervised activities in the community that include opportunities for socialization, community engagement, and volunteerism.

An extension of Friendship Community, which provides value through artistic expression to Artists with developmental and intellectual disabilities. Offers day studio art classes. Individuals may attend full or half day sessions in the Day Program.

A Supports Coordinator (case manager) works with each individual to develop and Individualized Service Plan (ISP) focused on what the individual needs to meet their maximum potential. Supports Coordination is available at no cost to registrants.

Also provides Intake and case management services to assess individual needs, assist individuals in accessing appropriate services, assess financial resources available and coordinate service delivery.

Funding options, natural supports and other community resources are explored and developed in order to assist individuals and their families achieve an everyday life.
