Offers assistance in accessing various resources through our Social Worker, Financial Counselor and Outpatient Care Manager. In addition, they have a Certified Spanish Interpreter on site to help facilitate optimal communication with patients, care and management of diabetes and other chronic illnesses, including transitioning from hospital to home through specialized office visits.

Coordinates all chronic disease programs sponsored and facilitated in partnership with other agencies; acts as a team leader for Injury Prevention, Tobacco, Oral Health, Asthma, Arthritis, Osteoporosis, as well as Cardiovascular programs. Chronic disease team nurses provide information to the public by presenting educational programs and are available for individual consultation.
